ABSTRACT

OBJECTIVE@#To observe clinical effect of acupuncture combined with conventional visual stimulation on cerebral visual impairment (CVI) in children aged 3-10 years and influence on the pattern visual evoked potential (P-VEP).@*METHODS@#A total of 60 cases of children aged 3-10 years with CVI were randomly divided into an observation group and a control group, 30 cases in each group. The children in the control group received conventional visual stimulation therapy, 1 month as a course of treatment. On the basis of the control group, the children in the observation group was treated with acupuncture at Baihui (GV 20), Jingming (BL 1), Taiyang (EX-HN 5), Sibai (ST 2), etc. 3 times a week, and the treatment was given 4 weeks continuously as a course. Both groups received 3 courses of treatment. The visual acuity and P-VEP improvement were compared between the two groups before and after treatment.@*RESULTS@#After treatment, the incubation period (P100-L) of the two groups was shorter than before treatment, and the amplitude (P100-A) was higher than before treatment (@*CONCLUSION@#Acupuncture combined with conventional visual stimulation can improve the incubation period (P100-L) and amplitude (P100-A) of P-VEP in children with CVI, and improve the best corrected visual acuity in children, the clinical effect is better than the conventional visual stimulation alone.

ABSTRACT

Objective:To investigate the effect of early enteral nutrition on ventilator-associated pneumonia in patients with Guillain Barre syndrome.Methods:This study was a prospective study.The study subjects were 36 patients diagnosed with Guillain Barre syndrome in the neuro-intensive care unit of our hospital from May 2011 to May 2017.Among them,18 patients received enteral nutrition support treatment within 24 hours after mechanical ventilation (early group),and 18 patients received enteral nutrition support treatment more than 24 hours after mechanical ventilation (control group).The two groups were compared with nutritional indicators,complications of enteral nutrition,ventilatorassociated pneumonia incidence,mechanical ventilation time,NICU residence time,APACHE Ⅱ score and mortality after treatment.Results:In the early group,the incidence of ventilator-associated pneumonia,mechanical ventilation time,NICU residence time and APACHE Ⅱ score after treatment were lower than the control group and the nutritional status index were better than the control group.Meanwhile,the incidence of complications of enteral nutrition such as diarrhea,gastrointestinal bleeding and stress hyperglycemia was relatively low.Moreover,the fatality rate was lower than the control group,but there was no statistical significance.Conclusions:Early application of enteral nutrition support therapy can improve the nutritional status of patients,reduce the incidence of ventilator-associated pneumonia,shorten the time of mechanical ventilation,reduce mortality and improve the prognosis of patients.

ABSTRACT

Objective To investigate the value of endoscopic assisted nasal jejunal tube placement and jejunum nutrition in treatment of critically illed patients. Methods This study was a prospective study. 56 patients with severe cerebrovascular diseases in the department of NICU from May 2014 to May 2017 were enrolled in this study. All of the patients were found to have an increased gastric remnant within 72 hours after admission (>100 ml). In the nasal jejunal group, 28 patients received nasal endoscopy and nasal endoscopic feeding, and 28 cases were treated with routine nasogastric tube and enteral nutrition. Comparing the two groups of patients with gastrointestinal complications (including vomiting, gastroesophageal reflux, abdominal distension, diarrhea, stress ulcer incidence) and nutritional status parameters (including serum albumin, prealbumin, hemoglobin), incidence of hospital acquired pneumonia and inflammation indexes (including WBC, PCT and CRP), stay at NICU time, NICU expenses, GCS score and mortality rate. Results The success rate of nasal intestinal tube placement under nasal endoscopy guided wire was 100.0%. The nasal jejunal nutrition status parameters were better, NICU check-in time was shorter, NICU expenses were lower, and the inflammation index was lower than the control group. There were no significant differences in the incidence of gastrointestinal complications, the incidence of hospital acquired pneumonia, the GCS score and the fatality rate between the two groups. Conclusion For the patients with severe gastric remnant increase in NICU, it is worth recommending that naso jejunal tube be inserted into the jejunum by endoscopy and using jejunum nutrition.

ABSTRACT

<p><b>BACKGROUND</b>In county-level tuberculosis (TB) dispensaries in China, the accurate diagnosis of sputum smear-negative pulmonary tuberculosis (SNPT) needs to be improved by developing and validating clinical and radiographic predictors.</p><p><b>METHODS</b>The study was conducted simultaneously in three counties per province in Chongqing Municipality and Liaoning Province in China between May 2005 and May 2006. A total of 432 new SNPT patients who are HIV-negative and more than 15 years old diagnosed by expert panels in county-level TB dispensaries were recruited. Their sputum samples were collected for culture before anti-TB treatment, and the treatment outcomes (changes of X-rays) were followed up at the end of the 6th month.</p><p><b>RESULTS</b>Of the 432 SNPT patients, sputum culture positive (9.7%) or culture negative with good changes of X-rays at the end of the 6th month (73.6%) was validated as SNPT. Four predictive variables were associated with validated SNPT in the multivariate logistic regression model: age ≤55 years old (odds ratio (OR) 5.66; 95% CI 2.69-11.91), >60 days of cough (OR 3.73; 95% CI 1.10-12.65), ≥10% of pulmonary consolidation in the lungs (OR 5.40; 95% CI 2.90-10.06), and pulmonary consolidation in the upper lobe anterior segment (OR 3.00; 95% CI 1.57-5.72). The area under the receiver operating characteristic curve of the model was 0.77 (95% CI 0.71-0.83).</p><p><b>CONCLUSION</b>Four predictors of clinical and radiological characteristics that had a good diagnostic performance of SNPT deserve to be recommended as index indicators of SNPT diagnosis in county-level TB dispensaries in China.</p>

ABSTRACT

Objective To measure the proximal femoral parameters which can provide anatomic evidence for the design of internal fixation components for intertrochanteric fractures of femur.Methods Femoral speci- mens were harvested randomly from 120 healthy adult cadavers(left 60,fight 60).The neck shaft angle,greater trochanter slope length,tilt angle of greater trochanter,axial length of head and neck,lengths of upper and lower borders of the neck,and the minimum transverse diameter of the neck were measured.On the basis of the anatomic study,a two-claw plate was designed to treat 145 cases of femoral intertrochanteric fractures.Results The femoral neck shaft angle was 128.59??6.31?,femoral greater trochanter slope length was(5.5?0.58)cm,tilt angle of femoral greater trochanter was 42.76??5.20?,and axial length of head and neck was(9.42?0.38)cm. There was a correlation between the parameters.All the patients were followed up for a mean time of 23.6 months. The fractures got clinic union in 3 to 6 months.Two cases experienced detachment of claws and hooks but their final outcome was not affected.Three cases suffered coxa vara.All the other cases obtained normal motion function of hips and normal neck shaft angle.No breakage of claw,hook or nail was found in them.Conclusions It is necessary to design an internal fixator that can fit the anatomical features of Chinese femurs in the treatment of intertrochanteric fractures of femur.The two-claw plate designed by us is a good attempt to improve the clinical effect.
